Full removal question

So to the board experts who know more than I do, if one were to go forward with full removal can you explain if it's substantially different than the free fat removal surgery.

I think from my limited understanding, the PMMA attaches like glue to the Bucks fascia which is the "normal" base layer (in that video where they cut the fat away from, ends up looking cleanly separated). So if it attaches more than fat does basically they would have to carve away/ peel away bits stuck to the Bucks that don't have to do with the fat removal. My ultimate question, would the procedure really be much harder than the fat removal here? Ok worst case the PMMA is stuck like glue to the Bucks and they can't separate it, but they shave everything down to a few millimeters thick layer and reattach the dermis skin and all that's left of PMMA is a layer that's not noticeable. Seems relatively straight forward for removal (comparativly speaking, obviously realizing this is a significantly invasive operation, but still the surgeon in that vid doesn't seem all that troubled by the procedure complexity or healing outcome). Anyone who has more knowledge see what I'm missing?
